The Health Promotion Outreach Programme is an ongoing programme of work, delivering Vital 5 health checks, vaccinations and health promotion in community settings. Lot 1: This core health promotion outreach team will conduct Vital 5 health checks and Making Every Contact Count (MECC) conversations to enable holistic whole person assessments and onward referrals as required.
