Care at Home - Community Support Services Flexible Framework Agreement Lot 1: Older People / Children & Young People Support Services Citizen Cluster 1 – Older People / Children & Young People Support Services Individuals with complex care needs Individuals with physical disability Individuals with dementia Individuals who are frail and/or vulnerable Individuals with addiction(s) Children and Young People with complex care needs Individuals with sensory impairment Individuals who are neurodivergent – diagnosed or undiagnosed The Service shall operate 365 days per year between the core hours of 0700 hours and 2200 hours. Children and Young People with eligible care and support needs shall also fall within the scope of Citizen Cluster 1. Further information regarding the anticipated profile of this element of the Service shall be provided within the procurement documentation, where available. Lot 2: Mental Health Citizen Cluster 2 – Mental Health Support Services Individuals with a diagnosed Mental Health Illness Individuals with a diagnosed Mental Health Illness with complex care needs Individuals with a diagnosed Mental Health Illness with physical disability Individuals with a diagnosed Mental Health Illness with dementia Individuals with a diagnosed Mental Health Illness who are considered vulnerable Individuals with a diagnosed Mental Health Illness with addiction(s) Individuals with a diagnosed Mental Health Illness and a sensory impairment Individuals who are neurodivergent – diagnosed or undiagnosed The Service shall operate 365 days per year between the core hours of 0700 hours and 2200 hours with overnight support where required. Lot 3: Learning Disability Support Services Citizen Cluster 3 – Learning Disability Support Services Individuals with a learning disability Individuals with a learning disability with complex care needs Individuals with a learning disability with physical disability Individuals with a learning disability with dementia Individuals with a learning disability who are 65-year-old or over Individuals with a learning disability with addiction(s) Individuals with a learning disability and a sensory impairment Individuals who are neurodivergent – diagnosed or undiagnosed The Service shall operate 365 days per year between the core hours of 0700 hours and 2200 hours with overnight support where required
