Chances Transitional Supported Living Ltd implement Pathway Plans in partnership with each young individual taking their unique circumstances into consideration as and when necessary. Our comprehensive plan includes information relating to: Housing Education Training and Employment Health, including Sexual Health Finance Independent Living Skills These plans will be reviewed and amended at regular intervals. We will ensure they are being followed and are adapted to motivate the young individual to achieve these set goals. Chances Transitional Supported Living Ltd provides shared and semi-independent supported accommodation for looked after children and/or care leavers for children aged 16 to 17 and young people 18 to 25 years, including those with children. Chances Transitional Supported Living Ltd's purpose is to provide a caring, safe, and homely environment that promotes empowerment, independence, and choice, whilst enhancing individuals' daily living skills.
