Tender description

The requirement is for provider to deliver care and support at the Emergency Accommodation and Assessment Unit, located at Shawe Road in Urmston. The service supported Adults in Trafford with a Learning Disability and/or Autistic Spectrum Condition, who require a short term emergency placement under the following circumstances: - Breakdown of current care arrangements, whether in a supported service or with family/carers. - Emergency respite requirement to prevent carer breakdown. - Transition to (and identification of) longer-term placements where required, including as a requirement for assessment of need in a supported setting. - Facilitation of hospital discharge before returning home or progressing to an alternative accommodation option. The service is registered as a Care Home with the Care Quality Commission (CQC) and the Provider will be expected to ensure they are appropriately registered in line with this. The intended length of stay is a maximum of 12 weeks.
