North West London Primary Care Enhanced Community Services

Tender description

The North West London Enhanced Services Single Offer provides a range of community-based services for patients registered with Hounslow GP practices, delivered through GP practices and Primary Care Network (PCN) hubs. The services are designed to provide timely, high-quality care closer to home, reduce avoidable hospital activity and improve patient outcomes. The offer includes respiratory and cardiovascular diagnostics, mental health support, phlebotomy, wound care, and specialist diabetes management. Services will be delivered by appropriately trained staff in accordance with evidence-based practice, clear referral pathways, and robust clinical governance arrangements. The offer promotes equitable access, early diagnosis, prevention, proactive management of long-term conditions and integrated multidisciplinary working, with a focus on reducing health inequalities and providing personalised, patient-centred care. Lot 1: - the services to be provided are Primary Care Enhanced Services to patients that are registered with a GP surgery in the London Borough of Hounslow;<br/>- an award has been made following direct award process A;<br/>- the lifetime value of the contract is £21,727,946 excluding VAT across all the Hounslow five Primary Care Networks (PCNs) (C/O NHS West London NHS Trust)as follows:<br/>1. Brentworth PCN £3,779,950<br/>2. Chiswick PCN £2,458,579<br/>3. Feltham and Bedfont PCN £5,342,688<br/>4. Great West Road PCN £4,213,421<br/>5. Hounslow Health PCN £5,933,308<br/>- the dates between which these services are to be provided is from 01 April 2026 to 31 March 2028 (24 months). (Note that this is a retrospective start date).
