Tender description
NHSE Health and Growth funded project piloting a new approach for caring for people with multiple LTCs. The provider will adopt a new way of working to improve and understand how we can best configure services around the needs of people with multiple LTC's (with a focus on the working age population from September within the cohort, still focusing on our priority PCN areas). Lot 1: This notice is confirmation of an award having been made due to an urgent circumstance. The Contract Authority; NHS West Yorkshire Integrated Care Board has awarded a contract under the PSR Urgent Award process. The authority is publishing this notice in Find a Tender in accordance with the NHS Provider Selection Regime.<br/><br/>Contract Period: 01/09/2025 - 31/03/2026 - with a 12 Month permissible extension.<br/><br/>Contract value: £83,333
