Pre Placement Agreement for the Provision of Support for Adults with Disabilities to Live in Their Communities

<p>The Service will be available for adults aged 18 or over who have a disability, and have been assessed as eligible for a service from Adult Social care (&lsquo;disability&rsquo; means a person aged 18 and over who is resident within the City of York who has been assessed by Adult Social Care as having a physical or mental impairment that has a &lsquo;substantial&rsquo; and &lsquo;long-term&rsquo; negative effect on their ability to do normal daily activities). The Service will provide care and/or support for customers of the Council&rsquo;s Adults Services who have a disability. The services defined below may be delivered by one Provider or a variety of Providers to form the Customer&rsquo;s package of support. All bidders must register their interest for the Yortender scheme titled &ldquo;Pre Placement Agreement for the provision of Support to live in their Community for Adults with Disabilities&rdquo;.
