Award notification: Rochdale Adult Care Services and the Heywood, Middleton and Rochdale Clinical Commissioning Group (CCG) are committed to reforming the Care at Home services within the Borough of Rochdale and ensuring they are equipped to meet the changing and growing demand from the population it serves. We are seeking to commission a Care at Home service that is focused on delivering outcomes for individuals who live in the Borough of Rochdale. The Service specification outlines our vision for the delivery of these services, the key features of the service and the outcomes we expect it to achieve. There will be an enhanced focus on hospital admission avoidance, behaviour change and self-care. This is an exciting opportunity to provide a service to people in a new and flexible way to maximise their ability to live independently and safely, in their own home within the community. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Middleton Neighbourhood Area. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Heywood Neighbourhood Area. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Rochdale West Neighbourhood Area. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Rochdale West Neighbourhood Area. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Rochdale East Neighbourhood Area. The service will cover all client groups including but not limited to: adults over and under 65, personal and social care and support with non-complex primary support needs of dementia, mental health conditions, physical disabilities, learning disabilities, sensory impairments, and substance misuse aged 18 or over. This lot will focus on the Pennines Neighbourhood Area.
