Provision of Core Community Services, supporting transformation of Primary Care and Out of Hospital Care in Wandsworth Lot 1: The notice relates to Community Health care services. This notice is an intention to award a contract to the existing provider following Direct Award Process C. The lifetime Value of the contract is £65,072,790 (£32,536,395 per annum). The contract term will be 1 year with the option to extend for 1 year. The contract will start on 01/04/2027 and end 31/03/2028 with an extension starting 01/04/2028 to end 31/03/2029
