Request for Information: Population Health Management, Population Segmentation and Risk Stratification Platform NHS South West London Integrated Care Board (SWL ICB) is carrying out preliminary market engagement ahead of a potential procurement of a Population Health Management (PHM), Population Segmentation and Risk Stratification platform, expected to be conducted through the G-Cloud 15 framework (RM1557.15), Lot 2b Software as a Service. This Request for Information (RFI) asks suppliers to describe their current capability against the full draft SWL PHM Service Specification. Please note that only suitable suppliers on Lot 2b Software as a Service of G-Cloud 15 will be invited to submit a tender in the event the ICB decide proceed with the procurement.
