End of Life and Palliative Care Services - Inpatient Care The service will provide a 20 bedded inpatient unit for terminal care, symptom control, assessment and crisis care for individuals who have a progressive terminal condition with specialist palliative care needs. End of Life and Palliative Care Services - Community The service will deliver a community-based palliative and end of life care services for individuals aged 18 and over requiring domiciliary end of life care, complex symptom management and assessment of their changing condition and/or disease progression. Palliative Care Personal Carer Support Service The service will support the needs of family and carers of those who require palliative and end of life care by enabling them to have regular breaks and respite from the caring role at times to suit them. Approximate lifetime value of the contract is £2,273,542 Contract term - 05/05/2026 - 04/05/2027
