Tender description
Aims to deliver more consistent care in patient’s own homes, in the GP practice and within two multi-functional Integrated Care Centres at St Charles and Violet Melchett. The model has been constantly evolving with the long-term aim being to provide holistic integrated care that meets the needs of patients with Long Term Conditions and complex needs in West London.
