Tender description
"NHSE is looking to procure capability/tooling solution, associated licensing, plus implementation support for proactive, predictive, and prospective 'case finding' solution". Case finding involves better identifying individuals who may have specific health conditions, high healthcare utilisation, poor outcomes, or worsening conditions and by enabling this it allows earlier intervention and support. It includes, but is not limited to, capabilities to cohort, segment, and risk stratify populations to enable earlier intervention and support.
