Social Prescribing service in the city of Leeds. Service to support primary care to proactively manage health and wellbeing and address wider detriments of health for people in their communities. The aim of the service is to connect people to services and activities in their community to benefit overall health and wellbeing. Lot 1: The Contract Authority; NHS West Yorkshire Integrated Care Board intends to award a contract to an existing provider following Direct Award Process C. The authority is publishing this notice in Find a Tender in accordance with the NHS Provider Selection Regime.
