NHS Sheffield CCG - Physical Health Outcomes - SMI Service High level outcomes: • National SMI annual physical health check targets are improved and met in Sheffield. • GP practices will be able to report an increase in the number of SMI annual physical health checks completed and report a positive experience of the additional support provided by the service. The contract value stated below is for the full 3+2 year period - Annual value is £40,000 • Covid, flu and pneumonia vaccination rates for people with SMI will increase.
