Nutrition and Dietetics Service in West Hertfordshire

NHS Herts Valleys Clinical Commissioning Group is seeking providers interested in delivering a nutrition and dietetics service in West Hertfordshire. The community nutrition and dietetics service provides care to patients with dietetic related conditions in the community through the creation of a management plan, monitoring of symptoms and teaching self-management in order to reduce need for additional care and maximise quality of life. Qualified and accredited dietitians will operate the service within community clinics, patients’ homes, care homes, etc. The service may also operate in an acute setting, providing specialist dietetic input in a multi-disciplinary team if necessary. Total contract value for 3 years is 3 348 794 GBP including VAT. The community nutrition and dietetics service provides care to patients with dietetic related conditions in the community through the creation of a management plan, monitoring of symptoms and teaching self-management in order to reduce need for additional care and maximise quality of life. Qualified and accredited dietitians will operate the service within community clinics, patients’ homes, care homes, etc. The service may also operate in an acute setting, providing specialist dietetic input in a multi-disciplinary team if necessary. This nutrition and dietetics service includes: — acute and community outpatient dietetic activity for both adults and children, — home enteral feeding service for adults and children, — home visits for appropriate patients, — care home visits, — community hospital visits, — Tier 3 obesity management service for adult, — education for health and social care staff, — provision of patient materials, — advice and guidance for health and social care staff. Acute inpatient dietetic services are not included in the scope for this service. However, the provider will work closely with such services to ensure a seamless transfer of patient care and appropriate MDT working.
