Description
The Norfolk and Waveney NHS Integrated Care Board and its partner stakeholders is seeking a Partnership Development and provision of a Risk Stratification / Population Health Management (PHM) Services Digital Solution to provide bespoke data analysis, technical solutions and digital platforms to underpin their PHM strategy and support the prioritisation and delivery of interventions. N&W ICB's overall Population Health Management strategy requires analysis at various levels and enable access to and use of PHM information at all levels, across NHS and wider partners within the Integrated Care System (ICS) to support a strategically aligned approach to Population Health. The PHM digital solution being sought must be secure, complying with all Information Governance requirements.
