Adults with Learning Disability Supported Housing Schemes

Description

The services are for adults with learning disabilities living independently in self contained flats within the community. There are three supported housing schemes in Torfaen that will fall under this contract, Clos Y Fran, Somerton House and Ty'r Hen Ysgol. Each contract will comprise of an agreed number of block hours and also a sleep-in member of staff at each scheme.
