Social Care Wales is seeking a supplier to design and develop a coherent, evidence-informed and ready-to-launch Multi-Disciplinary Team (MDT) support offer for local authorities across Wales. Through research, stakeholder engagement, co-design, mapping of existing provision, testing and feasibility assessment, the successful supplier will create a practical, scalable and sustainable support model that complements existing improvement activity, responds to current and emerging system challenges, and provides clear implementation pathways for national rollout by autumn 2027.
