Description

Provision of an Integrated Urgent Care Centre including a GPOOH service for the population of Northern Staffordshire. North Staffordshire and Stoke on Trent CCGs have undertaken the design of a new service model for an Integrated Urgent Care Centre inclusive of a GPOOH provision for the population of Northern Staffordshire, this is in line with best practice nationally and aims to reduce the number of admissions into acute services. This is a service to: 1) Stream all self-presenting patients arriving at the ED at the Royal Stoke to determine whether they have a need that can be met in a primary care environment, this in-turn will aim to reduce demand on ED services, 2) Provide access for patients with an identified primary care medical need who initially present at ED, 3) Provide urgent access to a GP during the out of hour's period for patients assessed with a primary care need by the NHS111 service. This is a 3-year contract.
