Under the Light Touch Regime, the Council has designed its own procedure. The contract opportunity is divided into five lots which reflect the Services to be delivered under the Platform. The Lots are 1. Home care; 2. Live-In Care, 3. Continuing Health Care (CHC) and Complex Care for Adults and Continuing Care (CC) and Complex Care for Children; 4. Bridging Services; and 5. Extra Care. This notice advises of the successful providers that have been added to the platform to date. Further notices will be published as new providers join the platform. There is no limit to the number of providers who can be awarded onto to the Platform insofar as the requirements of the Authority are met. To provide the ability to commission in the future, the Council may choose to add additional Lots onto the Platform during its lifetime. These lots may include Reablement, Intensive Bridging or similar described services. New and existing Platform providers can apply for any additional lots under the Platform Agreement. The Authority reserves the right to modify the platform during its operation. The Authority also reserves the right to terminate the operation of the platform at any time. Please refer to the Tender documents for further information. Lot 1: Lot 1 - Homecare Long term intervention of care within an individual's home to support people in accordance with their care plan. Additional information: The estimated aggregate value for Lot 1 over 20 years (10+10) is £476,068,579 this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative. Lot 2: Lot 2 - Live in Care Services 24/7 care within an individual's home to support people in accordance with their care plan. Live-In Care includes the following sub-lots. Lot 2 a. Short term Live-In Care are Live-In packages that are required for two or less weeks. Lot 2 b. Medium term Live-In Care are Live-In packages that are required for up to four weeks. Lot 2 c. Long term Live-In Care are Live-In packages that are required for five or more weeks. Lot 3: Lot 3 - Continuing Health Care (CHC) and Complex Care for Adults and Continuing Care (CC) and Complex Care for Children Lot No 3 be eligible, individuals must: • Live within Southampton City Council boundaries and be registered with a Southampton City GP practice. This is mandatory and both elements must apply for an individual to be eligible for this Service. • Be found to be eligible for Continuing Healthcare (CHC) or Continuing Care (CC) by NHS Southampton City place team within Hampshire and Isle of Wight ICB, or • Be found to be eligible for without prejudice or other temporary funding until assessment of CHC or CC eligibility is completed. Individuals who are eligible for CHC or CC may have a range of needs and must undertake an assessment as described in the tender documentation. Additional information: The estimated aggregate value for Lot 3 over 20 years (10+10) is £332,656,741 this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative Lot 4: Lot 4 - Bridging Services There is a need to support timely discharge from hospital in an enabling way which supports and maintains the wider health system, and to ensure that people experiencing a health crisis in their own home can be supported to stay at home and recover whenever possible. This Service aims to do this by facilitating a city-wide rapid discharge from hospital, maintaining people within their own home and avoiding hospital admission. Additional information: The estimated aggregate value for Lot 4 over 20 years (10+10) is £30,884,903this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative. Lot 5: Lot 5 - Extra Care Extra Care housing schemes are a type of 'housing with care' where residents live in their own homes, with their own tenancy. Some residents are likely to have individual packages of care set up to meet their needs. There will typically be a staff presence on site 24 hours per day. Extra Care is mainly targeted at those over 55 but may also be used to meet the needs of younger adults with physical disabilities, learning disabilities or mental health conditions. Additional information: The estimated aggregate value for Lot 5 over 20 years (10+10) is £64,736,932 this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative.
