Emergency Department Well-being and Home Safe Service Lot 1: The purpose of the Emergency Department and Well-being home safe service is to: - improve well-being through non-clinical advice and support via clear communication with staff, patients and families - delivery of trusted and safe resettlement of vulnerable individuals at home - ensure patients have follow up well-being checks - provide a seamless link to local community support and a reduction in avoidable hospital admissions - improve experience whilst waiting in the Emergency Department - improve patient flow through the emergency department
