This model reduces the risk of isolation that can lead to an increase in physical and mental health needs, and often subsequently an increase in funded support. The Authority is seeking a strategic partnership with a Provider(s) to work with through a relational commissioning approach and to establish the achievement of improved outcomes for Wandsworth residents.
