This is the contract award notice for the NHS Southport and Formby CCG and NHS South Sefton CCG — Community Anticoagulation Service. This is a re-award notice due to delayed mobilisation from the appointed provider and the notice designed to inform that the new contract start date is 3.6.2019; the new contract end date is 31.5.2022 with the option to extend for 1 x 2-year period from 1.6.2022. The overall aim of the service is to provide a consultant-led, community-based anti-coagulation service which will improve patient access to initiation of anti-coagulation therapy and provide monitoring of warfarin across various clinic sites, and in patients' homes for housebound patients. The service will be delivered to all appropriate patients registered with GP Practices in South Sefton and Southport and Formby and will ensure equitable access and quality of service to the entire population group. The service will be responsible for making specialist decisions regarding initiation of warfarin for patients referred into the service and provide ongoing monitoring and review of these patients. This will include regular reviews of TTR with recommended actions communicated to the patient's GP. In addition, the service will provide clinical expertise and input if changes to anticoagulant therapy is required (the service provider will manage any changes to therapy), implement bridging plans, and manage patients prescribed long term low molecular weight heparin (LMWH) therapy.
