Provision of community services for patients within Sutton localities. These service support people to stay well in the community, manage their own health with the right support and avoid stays in hospital. Services work across a wide range of health centres, hospitals, GP surgeries, children’s centres, local council facilities and in community settings — including in people’s homes. Scope of these community services is intended to include: — single point of access and referral; — PCN community nursing teams; — community nursing — tissue viability; — community nursing — continence; — MDT clinical service; — specialist nursing — tier 3 diabetes; — specialist nursing — heart failure; — specialist nursing — respiratory; — Palliative Care Coordination Hub; — dietetics incl. Dysphagia for adults with LD; — adult SALT; — neurology incl. Parkinson’s; — podiatry/foot protection; — MSK incl. outpatients physio and rehab; — speech and language therapy; — physiotherapy; — occupational therapy; — community dietetics; — specialist nurse for looked after children; — specialist school nursing; — Enhanced Care Home Support Team; — Sutton Health and Care at Home; — domiciliary physiotherapy; — community falls prevention and OPARS; — early supported discharge.
