Description

Providers shall work with people with learning disabilities to help them achieve their personal outcomes and aspirations in such a way that maximises their choice and control. Service delivery will be flexible to respond to how each person wants their service delivered to them to achieve their outcomes, which the Authority has agreed with them. Support levels will vary and services will need to be highly flexible and responsive to changing needs; to provide support that is appropriate to each person's requirements and aspirations and which effectively promotes their well-being and independence. The Service specification objectives shall be met by: — The successful application of Person Centred Planning (PCP) principles to support residents with a learning disability to; — Establish choice and control over their lives including the decisions that affect them and the services that support them; and — To achieve and maintain a strong sense of well-being and improved quality of life that is meaningful to them and which supports each resident to improve their health; Enabling people with learning disabilities to: — Pursue their lives the way they want through the positive management of risks; — Support them to retain skills already attained while learning new skills promoting their independence; — Build their confidence to increasingly support themselves in their home; and — Develop their capacity to be active and valued citizens in the communities where they live; The development of a highly motivated and skilled workforce that can: — develop an acute and strong understanding of how each resident they support communicates their desires, choices, likes and dislikes both through verbal and non-verbal communication; — Use this understanding to support each resident in deciding what they want from their support and services to help them achieve their outcomes and to help other professionals and agencies plan and provide the other services they need and want to use; — Facilitate GPs, dentists, housing and other services in providing their service in a way that is appropriate to the requirements and needs of each resident they support and which builds each resident's confidence in accessing health and other services; — Offer creative, innovative and effective solutions and options to support achieving the outcomes of each resident they support and in addressing challenges that may affect each resident's well-being, independence and safety; — Effectively respond to emerging good practise and changes in legislation in the delivery of support and care to people with a learning disability and who use training and personal development to continually improve the quality and standard of the support they provide.
