This is a Partnership Agreement between East Sussex County Council and East Sussex Healthcare Trust to improve the outcomes for young children and their families through the delivery of the 0-5 Healthy Child Programme (HCP). Additional information: This collaboration agreement had an initial period end date of 31 March 2023 and provided for a three year extension option up to 31 March 2026. The parties have agreed to extend the agreement for the three years and the contract value has been amended to reflect this.
