North of England Commissioning Support are working for and on behalf of NHS England, Yorkshire and The Humber who propose to procure an Alternative Provider Medical Services Contract in the following lots: Lot 1 (1 Site): Sheffield City GP Medical Centre — Rockingham House, 75 Broad Lane, Sheffield, S1 3PB. The actual patient list size as at July 2015 for this medical centre is 4 868 (3 966,06 weighted patient list). Please Note: Sheffield City GP Medical Centre is not a pre-determined site; therefore potential bidders will be required to advise in their bid how they intend to deliver services from a central location within Sheffield City Centre OR from the current premises. Lot 2 (4 Sites) ‘The Clover Group’: Main Site: Darnall Primary Care Centre, 290 Main Road, Sheffield, S9 4QH. Branch Site: Highgate Surgery, Highgate, Tinsley, Sheffield, S9 1WN. Branch Site: Jordanthorpe Health Centre, 1 Dyche Close, Sheffield, S8 8DJ. Branch Site: Mulberry Practice, 1 Mulberry Street, Sheffield, S1 2PJ. The actual patient list size as at July 2015 for these 4 sites is 16 655 (16 857,91 weighted patient list). Please Note: The Darnall Primary Care Centre is a pre-determined site therefore the successful bidder will be required to deliver services from the advised site address. The Highgate Surgery is not a pre-determined site and therefore potential bidders are required to advise in their bid how they intend to deliver services from locations within close proximity to the existing practice premises OR from the current premises. Jordanthorpe Health Centre is a pre-determined site therefore the successful bidder will be required to deliver services from the advised site address. The Mulberry Practice is not a pre-determined site and therefore potential bidders are required to advise in their bid how they intend to deliver services from locations within close proximity to the existing practice premises or from the current premises. Lot 3 (1 Site): Westwood Health Centre, Pelham, Manton, Worksop, S80 2TR. The actual patient list size as at July 2015 for this Health Centre is 3 763 (3 850,07 weighted patient list). Please note: the Westwood Health Centre is a pre-determined site therefore the successful bidder will be required to deliver services from the advised site address. The aim of the service is to deliver essential, additional and enhanced services to registered patients of each of the medical centres. The successful bidder will be required to provide GP or nurse led primary medical care services. In addition, the successful bidder for the Sheffield City GP Medical Centre will be required to provide a home visiting service for those patients presenting with an urgent clinical need, who live within the Sheffield boundary but are registered with a practice outside of NHS Sheffield CCG boundary in accordance with GP Choice requirements and a violent patient service, for patients that have been subjected to immediate removal from a practice's list, to provide a stable environment for the patient to receive continuing health care, addressing any underlying causes of aggressive behaviour and providing a safe environment for the individuals involved in delivering that treatment. In addition, the successful bidder for the Westwood Health Centre will be required to provide a home visiting service for those patients presenting with an urgent clinical need, who live within the Bassetlaw boundary but are registered with a practice outside of NHS Bassetlaw CCG boundary in accordance with GP Choice requirements and a violent patient service, for patients that have been subjected to immediate removal from a practice's list, to provide a stable environment for the patient to receive continuing health care, addressing any underlying causes of aggressive behaviour and providing a safe environment for the individuals involved in delivering that treatment. The basic principles of the open procurement route will be forwarded for this procurement to test the capacity, capability and technical competence of Tenderers in accordance with Part B regime of The Public Contracts Regulations 2006 (as amended). Regulation 120 of the Public Contracts Regulations 2015 provides for a temporary exemption for NHS healthcare procurements before 18.4.2016.
