Business and domestic software development for Weight Management and Complex Obesity Service Single Point of Access

Description

SNEE ICB, in collaboration with ESNEFT and local weight management providers and commissioners across the Integrated Care System (ICS), propose the creation of a Single Point of Access (SPoA) for all ICS weight management referrals. This is currently across the SNEE geography but is subject to change given future ICB changes, and therefore a scalable solution is needed. This new innovation would serve to risk stratify and triage patients into the most appropriate initial weight management service pathway, navigating the patient in a timely manner and based on clinical need to either a local weight management provider, an NHS digital weight management offers (Digital Weight Management Programme, Diabetes Prevention Programme, Type 2 Diabetes to Remission programme) or the ESNEFT WMCOS which includes Community Outreach for GLP-1 medication delivery. ## Contract award
