Description

The Learning Disabilities Floating Support (LDCS) service is a locality model (covering North/South Camden) that seeks to improve outcomes for children (14+) and adults (18+) with LD and a range of complex co-occurring needs, such as mental health, autism and behaviours of concern. It is preventative in nature, as the support prevents, reduces and delays the need for more intensive accommodation-based support. The service also contributes to meeting the Council's statutory duties under the Care Act and Section 117 of the Mental Health Act. The children's element of the service, (14- 25 year), applies the learning from the 'Closer to Camden' Commissioning Strategy for Children with Complex Needs, which cites the importance of an early intervention approach in reducing the need for residential placements and improving transition outcomes for young people. Seven priority areas were coproduced with key stakeholders and will be central to service outcomes, they include: • Structured support that promotes independence, choice and control. • Culturally competent and person-centred support. • Reducing social isolation and support to make friends through employment and shared activities • Improving health outcomes through the uptake of Annual Health Checks, cancer screening and healthy living choices, for example in addressing obesity. • Support to improve outcomes for young people transitioning from children to adult services. • Support that can flex and respond to the diverse needs of the people eligible for the service. • The support to be delivered by consistent and regular staff. • To procure two strengths based individually tailored services in the North Camden/ South Camden. • To include a transitional element for young people aged 14 to 17 years old. • To set an hourly rate that is realistic and competitive. • Ensure borough wide consistency in quality and delivery. • Create workforce capacity to address the recruitment and retention challenges providers are facing causing delays in residents receiving support. • Enable better co-ordination of referrals to create efficiencies and ensure the support remains person centred, culturally competent and strengths- based There are two Lots Community Support - North Lot 1 (North) Estimated weekly hours: 1055.
