Description

West Hampshire CCG Continuing Healthcare Team (and on behalf of North Hampshire CCG, South Eastern Hampshire CCG, Fareham and Gosport CCG and North East Hampshire and Farnham CCG), (the CHC Team) is awarding the tender for Fast Track Nursing Home capacity across Hampshire (excluding Isle of Wight, Southampton and Portsmouth), in order to improve the timeliness of provision of care for Fast Track eligible Service Users. The procurement is to implement a framework and to award the immediate provision of block purchased Fast Track beds. The framework will have a duration of 3 years plus 2 x's 12 month extension periods. Estimated total is for the total contract duration inclusive of extensions across the CCG's and is dependant on the number of Beds block purchased during the life of the framework. The framework will be in place for 3 years plus 2 optional 12 month extension periods.
