Thurrock Clinical Commissioning Group (CCG) is looking to award to a single Provider to deliver the proposed mental health integrated primary care model which will provide the support GPs require to manage people being discharged from secondary care and also those with complex needs that would best be addressed in primary care minimising the need to refer them to secondary care services.
