The Bristol Dementia Wellbeing Service delivers an integrated person-centred primary care-led model for people with dementia and their family/carers, at home, in care homes or other residential settings. This service is designed to offer comprehensive support with the primary aim of enhancing the wellbeing and quality of life for those affected by dementia and ensuring a timely diagnosis. The General Practitioner is the co-ordinator of care for people with dementia and their family/carers. The Bristol Dementia Wellbeing Service work alongside, providing a co-ordinated and integrated approach to assessment diagnosis and treatment and are responsive to the requirements of primary care, providing specialist advice, ongoing support, and specialist interventions. The service assigns a dedicated dementia practitioner and advisor to each GP practice, and proactive touch points are maintained with individuals every six months. Holding individuals on its caseload from diagnosis through to end-of-life, The comprehensive model offers diagnostics support, care home support, proactive, in reach support and training plus a portfolio of groups and activities for people and their carers. The service also provides a well-utilised help and advice line The service also offers a wide range of support, including personalised care plans, regular assessments, and access to a variety of services such as occupational therapy, counselling, and social activities. The service also provides valuable information and advice to help individuals and their families navigate the complexities of dementia care. The service work in partnership with the Alzheimer's Society, who provide a Community Development Team which works to address health inequalities faced by those affected by dementia and their carers and who work with Bristol's diverse communities, identifying and addressing barriers in accessing dementia services The service also provide dedicated dementia awareness sessions in local schools, to support the education of the younger generation in dementia and help to reduce stigma and offer wider community based support and education resources such as side by side volunteering opportunities and group activities. Lot 1: NHS Bristol, South Gloucestershire and North Somerset ICB are using this notice as an intention to award notice. Direct Award C has been used for the award of this contract. The existing provider is satisfying the original contract and will likely satisfy the proposed new contract. The services provided are the service specification are not changing considerably. The Commissioner is directly awarding, following direct award process C, a contract to Devon Partnership NHS Trust for the Bristol Dementia Wellbeing service in partnership with the Alzheimers society in accordance with the criteria outlined in Regulation (9) of the Health Care Services (Provider Selection Regime) Regulations 2023. NHS Bristol, South Gloucestershire and North Somerset ICB intent to award this contract to an existing provider following Direct Award Process C. The contract has been awarded for the period 01/04/2026 - 31/03/2028 This procurement has been carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners. NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (ICB) and Gloucestershire ICB are currently working together in a formal "clustering" arrangement to prepare for their merger into a single, new organisation (name to be confirmed), effective from 1st April 2027.
