Homeless and Rough Sleeper Service

INTENTION TO AWARD: This service supports Homeless and Rough Sleepers in Eastbourne, Wealden and Lewes. The service includes a GP clinic (up to 8 hours per week) and a community nursing service providing a range of outreach services. Lot 1: INTENTION TO AWARD: The existing provider continues to provide a satisfactory service and is likely to deliver satisfactory services within the contract requirements. There is no change to the services from previous years and delivery aligns with service requirements. Aims and Objectives of service The overall aim of the service is to consolidate primary care for homeless patients and rough sleepers and those in temporary accommodation within Eastbourne, Lewes and Wealden. This model will support integration of care directly addressing health inequalities and coordination. Aims To deliver a dedicated service in Eastbourne town centre and an outreach service in Lewes and Wealden for homeless and rough sleepers and those in temporary accommodation to help break down existing barriers and increase accessibility to mainstream health services. Objectives To support the integration of health, care and support services directly addressing health inequalities, collaboration and improving lives. The service will do this by a. Providing medical interventions b. Providing case management and care coordination c. Providing medical and risk management d. Proactively engaging with service users e. Providing personalised care f. Working in an integrated, collaborative and flexible way with other services that homeless people and rough sleepers' access g. Proactively testing their delivery model taking learning to support the evolution of the service ensuring patients ongoing needs are met Annual contract value: £235,825. Total contract value (2-years) =£471,650. Contract service dates = 23rd November 2026 to 22nd November 2028.
