Summary of the work Delivery of online Cognitive Behavioural Therapy (CBT) by BABCP accredited therapists directly in patient homes through a technological solution through online web browser, at home, via live, one-to-one, written communication. Performance data to be supplied monthly based on recognized clinical outcomes. Expected Contract Length 12 months with the possibility to exend by an additional 12 months. Latest start date Monday 1 April 2019 Budget Range Project funding will be discussed after award if appropriate. Project funding is set for 1 year. Why the Work is Being Done Through policy drivers, the Scottish Government is urging NHS Health Boards to increase access to evidence based services through the implementation of innovated new approaches that can be delivered at scale to large populations, especially to those who currently find it difficult to access treatment. This is an ongoing challenge in all areas of Scotland but particularly in rural and remote areas who continually find it difficult to recruit and retain staff in key clinical roles. Problem to Be Solved Increase capacity to deliver evidence based CBT therapy delivered by accredited therapists through a technological solution and provide an alternative means of engagement for patients who may experience a number of barriers to accessing treatment including geography, time constraints, physical difficulties or personal boundaries, lack of appropriate clinical staff or long waiting times Who Are the Users Adult patients ages 18 and above that suffer from moderate to severe psychological distress, this may include patients with long term health conditions. Patients need to either be referred to an online service or will self-refer using a portal provided by the supplier. Supplier will arrange an appointment and patients will engage in Cognitive Behavioural Therapy (CBT) over the internet, using a secure "therapy room" that is confidential and discrete. CBT treatment is accessed from service user's computer, tablet or smartphone and requires a low internet bandwith than VC methods. Early Market Engagement NHS 24 has had preliminary discussions with IESO Digital Health who currently provide an internet enable CBT Service to provider across NHS England. We have also consulted with NHS Western Isles in Scotland, who are considering a similar requirement. Work Already Done Discovery phase consisting of market and clinical research is complete. A test of change project has been discussed at Heads of Psychological Services, NHS Health Boards and MHAIST and is due to proceed in February 20019. Existing Team The supplier will be working with the Digital Mental Health lead based within NHS 24. In addition to this the supplier will be expected to work with selected territorial Health Boards to implement the service solution, this will include the clinical psychology departments and GPs. The supplier will need to co-ordinate with procurement and finance with in NHS 24. Current Phase Not applicable Skills & Experience • Have an existing solution with a proven track record and be able to provide a demonstration/case studies/customer reference/performance data. • Be able to demonstrate the BABCP accreditation of all therapists delivering care using the solution. • The CBT provided must comply with NICE guidelines with validated PHQ9, GAD 7 and PEQ recovery metrics monitored to ensure compliance. • The therapy website must have effective security controls and ID authentication in place for both patient and therapist. • Therapy should be delivered by means of typed conversation allowing full transcripts of sessions to be securely retained and accessed by the therapist and patient via the website. • The patient website account should also allow for messages to be delivered independently of scheduled therapy sessions. • ISO27001, ISO9001. • Secondary criteria - Website interaction and functionality for users, availability of scheduled therapy at evenings and weekends • ability to implement a solution for NHS 24 within a rapid timeframe, assistance with promoting the availability of the service with healthcare professionals and potential client groups. • The solution must be available on multiple internet enabled devices and operationg systems Work Location The work will be co-ordinate from NHS 24, East Contact Centre, Norseman House, South Queensferry, EH30 9QZ. However there will be a need to work directly with a minimum of 2 territorial Health Boards. These are yet to be identified. Working Arrangments Contract review and discussion can be conducted remotely although 1 or more on site visits may be required. Incidential expenses related to the marketing, communication and delivery of service (such as travel) will not be reimbursed and should be factored in to the overall price quoted. Security Clearance N/A beyond NHS clinical confidentiality standards and data sharing or any site visit in care providing locations. Provider will not access NHS systems. Additional T&Cs Disputes under the contracted will be settled under Scottish law No. of Suppliers to Evaluate 5 Proposal Criteria • Please describe your technical solution for providing internet based, live messaging, Cognitive Behavioural Therapy to patients? • Please provide assurance of your organisations competence and accreditation in relation to the operation of this service, including the therapists who will be working directly with our patients? • How will you ensure the quality of the therapy sessions deliverd to patients? How will you measure and report patient outcomes? • Please provide an example patient journey from referral to discharge. You should include referral pathways for both GP referral and self referral. • Please describe the data protection measures you have in place to maintain the confidentiality and security of patient information and therapy transcripts? • Please submit a project plan describing how you will implement the contract if awarded?
