Description

To support the publication of England’s Cardiovascular Disease Modern Service Framework the Department of Health and Social Care (DHSC) and NHS England are exploring collaboration opportunities with stakeholders from the voluntary, community and social enterprise (VCSE) and private sectors interested in supporting the Government’s ambition to reduce premature deaths from heart disease and stroke by 25% within a decade. We are asking interested parties to submit the below form detailing their proposed offer of collaboration. We are especially interested in initiatives focused on the prevention and management of heart disease and stroke, particularly in the most deprived groups. This includes initiatives around supporting people to lead healthier lives for example through healthy food environments and physical activity, public awareness of cardiovascular disease (CVD) and its risk factors, diagnosis of CVD and CVD risk, and rehabilitation.
