Description

The service provides non-clinical Care navigators to work in and alongside general practice to deliver a care coordination service, ensuring that patients at high risk of admission and / or with complex health and social care needs have a care plan in place (which is regularly reviewed) supported by proactive case management. This is an interim contract to allow for wider market engagement and to enable the service to be reviewed in the line the CCGs intention to procure a Multi-specialty community provider as part of its new integrated care system.
