Short Term & Reablement Services (SRS)

Description

This contract is a Call-off from the Help 2 Live at Home Framework (Reference: AS24587) SRS is a short- term enablement domiciliary care service offering timely support for individuals who may have an eligible Care Act need. The service will support individuals within their own home through the provision of packages of personal care whilst individuals await an assessment under the Care Act (2014). This service will not meet the needs of those individuals who require a primary health provision and or those who meet the criteria for an established health pathway. However, the Service Provider will be expected to be able to meet the needs of individuals in terms of supporting or administering medication in line with the prescriber's recommendations, and work alongside any health provisions which may wrap around the individuals core social care delivery to ensure a joined up approach to safe and effective discharge from hospital.
