Description

Provision of a new pilot person-centred Type 2 diabetes intensive lifestyle service involving clinically-sound interventions supported by life-style change programmes and intensive facilitation to reduce medication dependency to achieve remission or partial remission. Provision of a new pilot person-centred Type 2 diabetes intensive lifestyle service involving clinically-sound interventions supported by life-style change programmes and intensive facilitation to reduce medication dependency to achieve remission or partial remission. The service will provide and manage a healthcare campaign to encourage and support people across NW London with Type 2 diabetes to complete a course consisting of either a very low calorie diet (VLCD)/or a low carbohydrate diet in order to achieve significant health improvement and corresponding reduction in costs and medicines. The service will run for an initial 3-year duration from October 2019 until October 2022, with rights reserved for this to be extended for a further 2 years (5 years maximum) until October 2024.
