Description

NHS Hammersmith and Fulham CCG intends to contract with the Hammersmith and Fulham GP Federation and General Practices. These contractual arrangements will be underpinned by a Tripartite Agreement between Hammersmith and Fulham CCG, the GP Federation and General Practices for a single integrated contract of enhanced primary care services from 1.4.2018. This will bring a range of current primary care components into a single offer of aligned and simplified Primary Care Standards. These include current services in the Out of Hospital Services contracts. The contract is in the process of being developed but will primarily focus on 1. Improving patient experiences of care. 2. Improving clinical outcomes (e.g. Cardiovascular Disease, Respiratory Disease and Diabetes). 3. Financial Sustainability (Demand Management, Non elective emergency admissions). NHS Hammersmith and Fulham CCG intends to contract with the Hammersmith and Fulham GP Federation and with 30 GP practices in Hammersmith and Fulham, which will be underpinned by a Tripartite Agreement between Hammersmith and Fulham CCG, the GP Federation and General Practices. Currently primary care is commissioned in a fragmented way through a number of different contracts, with practices required to deliver against separate schemes. Further to Independent Panel recommendation in September 2017, the award of this contract can only be made to the Hammersmith and Fulham GP Federation and General Practice under a Tripartite Agreement for a single integrated contract from 1.4.2018. The services included within the contract are mutually dependent upon the core GMS/PMS/APMS Contract. However, many services may require delivery at scale for the benefit of practices and patients, with inter-practice referral arrangements in place. This mutual dependency means that the Enhanced Primary Care Contract may be legitimately commissioned exclusively from the Hammersmith and Fulham GP Federation as well as for the other reasons outlined below: The GP Federation has sign up from all General Practices in the borough using the same clinical system and facilitating full population coverage; It ensures continuity of service provision and would engender seamless transition to future delivery models in line with the direction of travel set out in the Primary Care strategy; This option would provide practices with additional scope to work at scale, in line with the FYFV, which would not be available if the CCG contracted directly with individual practices.
