Frailty Consultant Services across Brent and Harrow

Description

An integrated medical model for CLCH’s Rapid Response, Care Home Teams & Falls Services across the boroughs of Brent and Harrow. This model will enhance operational efficiency and improve patient safety and quality of care in the frailty and falls services across Brent and Harrow. The primary purpose of the Frailty Service is to be clinically accountable for the Integrated Rapid Response, Enhanced Care Home Team (ECHT), Integrated Case Management (ICM), and Falls Services across Brent and Harrow, while providing clinical leadership and support to the Rapid Response team. The consultant(s) will be responsible for delivering high-quality medical care, preventing hospital admissions, and managing complex cases in the community. This role requires taking full accountability for clinical outcomes, ensuring that the consultant(s) leads the service with clear responsibility for decision-making and patient care. The consultant(s) will work closely with multidisciplinary teams to ensure seamless service integration and optimal patient outcomes.
