Description
North Somerset Council is undertaking market engagement to inform future commissioning arrangements for a Home from Hospital and De-Hoarding Support Service. The Council is exploring opportunities to develop an integrated preventative support service that combines hospital discharge support with specialist de-hoarding and home environment interventions. The intention is to support residents whose health, wellbeing, independence or safety may be affected by their living environment or who require practical support to return home safely following a hospital stay.
