The Council requires a Managed Service for the Completion of Social Care Reviews, predominantly 6-8 week reviews, although it is envisaged that the Provider may be required to undertake other reviews, prior to and after 6 to 8 weeks. Reviews will be undertaken to a standard which is 2014 Care Act compliant. This may be the first opportunity to review a newly implemented package of support. The Review allows opportunity for a review of outcomes, both in terms of the service offer to sustain and support people to live as independently as possible, and to make the most effective use of what can be provided locally to support people’s independence, where that is appropriate. Additional or alternative care and support services may be commissioned if deemed necessary with the aim to provide appropriate support for the individual. Strength based and asset-based approaches and practice, which is a social work practice theory that emphasises people's self-determination and strengths should be utilised. The Council’s intention is to award two contracts, one contract to cover the West (Lot 1) and the other to cover the East (Lot 2). Providers may bid to provide one or both lots. This procurement is conducted is conducted under the OPEN PROCEDURE. Any Tenderer wishing to bid for this opportunity must submit their tenders by 5pm on the 31st May 2024.
