NHS Bassetlaw CCG sought to appoint a single provider for the provision of a social prescribing service as outlined in the service specification. The service is required to be based in a community setting in Bassetlaw. The contract will be for a term of 3 years with the opportunity to extend for 2 further years subject to mutual agreement between the successful bidder and the Authority. The maximum term of contract is consequently 5 years.
