Description

Provision of integrated primary care services from 1st April 2018 for 12 months with the possibility of a further 12-month extension. Integrated services include the following: — ambulatory blood pressure monitoring, — anti-coagulation monitoring and initiation, — case finding, — care planning & case monitoring, — complex common mental health, — coordinate my care, diabetes (high risk) and level 1 and level 2, — ECG, — homeless care, — near patient monitoring, — ring pessary, — severe mental health (level 1 and level 2), — severe mental health shared care prescribing, — spirometry testing, — wound care, — carers, — end of life care, — A&E frequent attendees, — asthma, — extended hours, ad, — locality plan. West London CCG is due to contract with the West London GP Federation and with local GP practices, which will be underpinned by a Tripartite Agreement between: — West London CCG, — the GP Federation, and, — General Practices. The services included in the contract are mutually dependent upon the core GMS/PMS/APMS Contract. However, many services may require delivery at scale for the benefit of practices and patients, with inter-practice referral arrangements in place. This mutual dependency means that the Enhanced Primary Care Contract may be legitimately commissioned exclusively from the West London GP Federation as well as for the other reasons outlined below: The GP Federation has signed up for all General Practices in the borough using the same clinical system and facilitating full population coverage; It ensures continuity of service provision and would engender seamless transition to future delivery models. This option would provide practices with additional scope to work at scale, in line with the FYFV, which would not be available if the CCG contracted solely with individual practices. This procurement approach has been agreed by an independent review panel convened on September 17 which has recommended the CCG’s contractual approach.
