Provision of a Community Care and Support Service (Domiciliary Care) to individuals living in Sandwell. People living within the Borough of Sandwell. The service will provide a broad range of personal care and support services aimed at enabling individuals to maintain maximum independence, support their well being and remain living in their own homes to enable them to remain in their own homes whilst preserving maximum independence and quality of life. The service will be provided to Adults (over 18 years of age). Service user groups may include: Older Adults, including those with Dementia, Individuals with Physical Disabilities, Learning Disabilities, Mental Health Issues and Sensory Impairments.
