Resilience, Inclusion, Strengths and Enablement (RISE)

Please refer to the attachment in this advert for further details and planned pre-tender activity. Further materials will be attached in the period leading up to the tender being published in July. The Resilience, Inclusion, Strengths and Enablement (RISE) service will provide prevention, early intervention and reablement support to adults with needs related to homelessness, mental health, drug or alcohol dependency, neurodivergence or learning disabilities. The trauma-informed, short term, service will help people to find and sustain a home, build practical life skills, improve wellbeing, develop emotional resilience and feel connected within their community. Cornwall Council are encouraging a collaborative approach by providers responding to this planned tender activity. There are three components to the integrated service model that people will typically access for up to 12 weeks: A) Daily Living and Inclusion – lower intensity, early stabilisation, drop-in and community support. B) Resilience and Enablement – higher intensity, assertive, community support. Enablement of self-care related to mental health. C) Homeless Crisis and Reablement – higher intensity supported accommodation. Reablement of self-care related to mental or physical health.
