A digitally enabled renal rehabilitation and self-management support service for people with chronic kidney disease (CKD) and cardiovascular-kidney-metabolic (CKM) conditions, utilising the Kidney Beam digital platform, associated clinical intervention programmes and health coaching support. The Service supports optimisation of long-term condition management, patient activation and improved clinical and quality-of-life outcomes, aligned with NHS Long Term Plan priorities. LEGAL JUSTIFICATION: This is a transparency notice notifying the market that NHS North Central London will be awarding a contract for a Digitally Enabled Renal Rehabilitation and Self-Management Support Service to Kidney Beam LTD.
