The vision for Extra Care is to enable people to continue to live independently for longer through the development of vibrant and balanced communities with onsite care that adapts to changing needs, through effective partnership working between registered landlords, service providers and wider stakeholders. To support balanced communities within Extra Care housing, each scheme aims to balance the following split in care needs: 40% high need (10 hours +); 40% medium need (5-10 hours); and 20% low need (less than 5 hours). The aims and objectives of Extra Care services are: To provide personalised care and support to people living in Extra Care Housing who have assessed eligible care needs under the Care Act 2014. To maximise the independence of people living in Extra Care Housing, including those with Dementia, enabling them to live well and independently, carry out the functions of daily living, and to achieve positive outcomes in respect of addressing their identified needs and their overall experience of the service. To promote and maintain residents' health and wellbeing, enabling them to overcome any anxieties, build resilience and increase their confidence to live independently. To support recovery from illness, through supporting timely discharge from hospital and preventing unnecessary acute hospital admissions and re-admissions. To reduce residents' social isolation, improve the maintenance and development of their social support networks, facilitate community access and enable them to feel confident living in their community. To meet the fluctuating and developing needs of residents. To reduce to need for onward moves to residential and nursing care.
