Description

The service will provide a hospice in Hillingdon that will include: — 10 inpatient palliative care beds, — a 24 hour consultant led telephone advice and support line, — provision of day care services. This will provide care for patients requiring complex acute symptom management, both malignant and non-malignant in nature. The model will address the immediate issues and ensure a rapid approach to address the lack of provision of inpatient hospice care and the potential risk to the 24 hour phone line. The service will reduce the inappropriately high proportion of acute hospital deaths and improve patient/carers experience. Harlington Hospice is considered the only viable local option at this time, having the capability to rapidly mobilise to run this new model as a pilot as well as being a key partner in the local integrated end of life services. Direct award of an interim contract of 12 months will enable the re-opening of 10 inpatient hospice beds at Michael Sobell House whilst the development of the Hillingdon Health Care Partners Integrated Care business case if progressed and a new hub is commissioned.
